We are seeking to recruit a Patient Navigator to join our Infected Blood Psychological Service. We are a team of professionals dedicated to providing expert emotional and practical support to those affected by infected blood. The service forms part of a national strategy to provide specialist holistic support to this overlooked community.

This sensitive role involves guiding clients through their psychological and social care needs with empathy, respect, and understanding. You will work closely with individuals and families to help them access appropriate services, empower their choices, and support their wellbeing.

Main duties of the job
  • The post holder will provide a high-quality navigation/care support service, which includes psychosocial assessments and interventions to beneficiaries of the Infected Blood Psychological Service (IBPS).
  • The post holder will lead on outreach programmes and support the overall development of the newly founded service.
  • The post holder will be essential in delivering a multi- disciplinary approach to supporting the beneficiaries of the service, and the HCCC.
  • The navigator element of the service will be provided on a large geographical area.
Person Specification
Training
  • Degree in Social Work (or equivalent qualification) or equivalent experience
  • Social Work Registration
Experience
  • Significant experience of working with adults and children
  • Able to demonstrate specialist knowledge of adults and children with complex needs or chronic conditions.
  • Knowledge of research and audit. Understanding of safeguarding.
Skills
  • Excellent written and verbal communication skills.
  • Evidence of continuing professional/personal development.
