Responsibilities

  • Deliver the Co-ACTION intervention to participants within a defined local area.
  • Build trusting relationships with those living with multiple long-term conditions and their carers.
  • Provide proactive follow-up, anticipatory care, and support through care transitions.
  • Work collaboratively with GP practices and community services to coordinate care.
