Mental Health & Wellbeing Practitioner

Location: Aberdeen

Salary: £24,618 - £25,944 per annum (£12.62 - £13.30 p/h equivalent)

Overview

Responsibilities
  • Deliver high quality, person-centred support to five adults within their own homes in the community of Nigg.
  • Demonstrate professionalism, safeguarding, reflective practice and continuous service improvement including provision of supervision to others staff and working closely with the Service Manager.
  • Collaborate with individuals, their families and multi-agency professionals to promote positive mental and physical wellbeing through preventative and early intervention approaches.
  • Support individuals during periods of distress sensitively developing coping strategies and accessing the right services at the right time.
  • Increase individuals’ life skills in line with their preferences and the Health and Social Care Standards, involving a variety of housing support needs and care at home assistance.
  • Promote communication enabling individuals to meaningfully participate in decisions and choices that matter to them, whilst expressing their own personality and talents.
  • Assist individuals to progress and review their recovery via a range of coaching approaches and toolkits.
  • Facilitate joint activities with individuals in our resource flat ensuring participation and inclusion for all.
