A healthcare practice in Woking is seeking an experienced Clinical Pharmacist Practitioner to join their team. The role involves patient-facing clinical practice, chronic disease management, and collaboration with a multidisciplinary team. You will provide on-the-day appointments, manage patient medications, and ensure proper referrals for investigations. Ideal candidates will have independent prescribing qualifications and strong clinical assessment skills.

The successful candidate will utilise their patient facingclinical practice and independent prescribing qualification to work alongsideour established clinical team to support our medicines management agenda managepatients with chronic disease and develop their skills in seeing and treatingpatients as part of our multidisciplinary team.
Dutieswill include the ability to evaluate treatment/interventions and provide carefor patients with undifferentiated and undiagnosed problems using clinicalassessment skills in the Primary Care setting, working as part of ourmultidisciplinary duty team as well as assisting with long term conditionclinics and medicines management for the practice.

This is an additional clinical pharmacist post within thepractice and we are seeking an experienced Clinical Pharmacist Practitionerwith sufficient skills, enthusiasm and commitment to fulfil and develop therole within our practice team.
Youwill work autonomously with an extended role (within own competency) providinga combination of on the day appointments, alongside medicines management,chronic disease management and pharmaceutical advice to the practicepopulation.
Youwill be responsible for seeing and advising patients in respect of theircontinuing medical and Pharmaceutical needs and developing your skills to seeand treat patients who wish to be seen on the day by a health careprofessional; initially diagnosing and treating patients presenting with minorillness. The Pharmacist practitioner will need to prioritise and triage theneeds of patients accordingly making any necessary referrals for investigationsin the appropriate manner. Patients with problems needing referral to secondarycare will be discussed with a GP before making a referral.
