The successful candidate will be based at Ludlow Hospital and work across the beautiful south‑west Shropshire as part of our community therapy team. Our pathways include supported hospital discharge, urgent and end‑of‑life referrals requiring a 24‑hour response and routine community therapy. This is a seven‑day service with weekend working on a rota basis.
