A regional NHS Trust in Carshalton is seeking a Band 7 nurse to join their Community Integrated Locality team. The role involves working closely with care homes to provide high-quality assessments and care plans for elderly residents. Candidates should possess a valid NMC Registration and experience in care home settings. To work as part of the Community Integrated Locality team and alongside staff in Identified Care Homes to provide a high quality, clinically robust, personalised assessment of all residents identified as high risk of conveyance to Hospital. Working in partnership with key staff members in both the homes and community teams to ensure each identified resident has implemented a clear care plan to manage their health needs, to include a clear pathway of how to contact Sutton Community Health services (SCHS) to prevent avoidable conveyance to hospital.

Responsible for the daily operational line management of the Community Liaison Team.
Work collaboratively with staff in the acute setting, developing processes to support safe, timely discharge to the community environment.
Responsible for ensuring each team member carries out effective and efficient high quality nursing assessment of patients, that will enhance and support the discharge processes.
Ensure the successful delivery of services key performance indicators.
