An exciting opportunity has arisen to be part of a new project delivering SACT in patients' homes and on a mobile cancer care unit – a 12‑month pilot for patients undergoing treatment for haematology/oncology conditions.
The band 6 nurse will be responsible for assessing, developing, implementing and evaluating family‑centred care in line with the project. Phase 1 of the pilot will deliver SACT treatments in patients' homes and phase 2 will administer SACT on a mobile cancer care unit.
You must have excellent communication skills and work well as part of a team, recognise limitations and use initiative.
The post holder must have access to a car and hold a full driver's licence.
The post holder will manage a caseload and be responsible for the assessment of care needs, planning, implementing and evaluating care for patients in the care‑closer‑to‑home programme.
The post holder will provide direct patient care and have detailed clinical knowledge of the specialist area. They will ensure that high standards of quality of care are delivered and maintained, and act as a positive role model within the department and in the community for staff, patients and carers.
The post holder will effectively contribute to the care‑closer‑to‑home project and ensure that the provision meets the needs of patients and families.
The post holder will enable nurses to acquire clinical and professional competencies by facilitating high‑quality education and staff development opportunities.
