We are looking for an enthusiastic, innovative and engaging Band 5 NMC Registered Nurse, who has a distinct passion for the delivering high quality patient care within community services.
You will be joining our Integrated Out of Hours LAP Team on a part-time basis to provide remote triage of calls and referrals made to the Integrated Out of Hours Community Nursing Team.
As a triage staff nurse you will be expected to have professional and clinical knowledge of community nursing. Whilst not providing regular face to face patient contact you will need to maintain an up to date knowledge of the clinical aspects of community nursing in order to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. To ensure to a high standard of care is provided, occasional delivery of care to patients in a community setting in order to maintain up to date clinical experience.
You will be based at Greyfriars Therapy Centre in Stafford. The shift you would be expected to work would be 5pm until 1am, with the expectation of some night shifts.
Making appropriate decisions following comprehensive remote triage to ensure the safe management, and allocation visits where needed, in response to calls and referrals received by the community nursing service.
