A healthcare provider in Wigton is looking for a Hub Coordinator to join their innovative team. This role involves acting as a point of access for referrals while delivering high-quality community care. Salary is £24,937 to £26,598 per year, with 30 hours available weekly. This position offers opportunities for contributing significantly to community health services.

We are absolutely delighted to announce the launch of a Specialist Services Hub within the Community Collaborative. This exciting new initiative will bring together several of our specialist services—including Specialist Palliative Care, Bladder and Bowel Services, Acquired Brain Injury, and Virtual Wards—under one unified team.
Initially, the service will run Monday to Friday, excluding weekends & bank holidays but there may be scope in the future to provide 7 day cover across services.
The Hub will act as a single point of access for all referrals, enquiries, and contacts related to these services, making it easier for patients and professionals to connect with the right care quickly and efficiently.
We are seeking enthusiastic, adaptable individuals to join us in developing and delivering this innovative service. You will be part of a collaborative team dedicated to creating a seamless and supportive pathway for our specialist services.

The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:
Standard Check: £21.50 + Administration cost of £5.90 = £27.40. Deducted from salary over a 4 month period or a one-off payment.
Enhanced Check: £49.50 + administration cost of £5.90 = £55.40. Deducted from salary over a 4 month period or a one-off payment.
