Our client in Central London is seeking a dedicated Locum Social Worker to join the Hospital Team. Key Responsibilities
  • Undertake assessments under the Care Act 2014 to facilitate timely hospital discharges.
  • Work closely with health professionals, families, and community services to develop effective discharge plans.
  • Apply strengths-based, person-centred approaches to ensure positive outcomes for service users.
  • Ensure safeguarding concerns are identified and managed in line with legislation and local procedures.
  • Maintain accurate case records and documentation in line with council and professional standards.
Requirements
  • Qualified Social Worker (Degree/DipSW/CQSW)
  • Current registration with Social Work England
  • Previous experience in hospital discharge or adult social care services
  • Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding protocols
  • Ability to work under pressure in a fast-paced, multi-disciplinary environment
