Job description
  • Be part of a pioneering team shaping the future of care delivery, Identify suitable patients in the acute setting using current pathways, supporting early supported discharge through the Hospital at Home service
  • Conduct community assessments as needed and collaborate with multidisciplinary team (MDT) members to develop personalised care plans
  • Monitor patients remotely in accordance with standard operating procedures, ensuring safe and effective virtual care
  • Apply evidence‑based clinical skills and utilise digital technology to deliver high‑quality care in patients' own homes
  • Manage your patient caseload effectively, ensuring smooth handovers of care and maintaining clear, timely communication
  • Develop and sustain seamless referral pathways to other agencies and specialist teams, fostering strong MDT collaboration
  • Work within your scope of practice and clinical competencies, adhering to professional policies, national standards, and Trust procedures
  • Contribute to ongoing service development and quality improvement initiatives to enhance patient care
