Qualifications

• Experience working with people who are homeless or rough sleeping.

Responsibilities

• Build strong communication and referral pathways across local services.
• Attend multi-disciplinary meetings and strengthen partnerships.
• Support individuals moving between areas including care handovers.
• Provide culturally informed engagement for diverse communities.
• Hold short-term cases for successful service engagement.
