An organisation supporting children and families is seeking a dedicated Case Manager to provide high-quality, trauma-informed support for children and young people in foster care. In this permanent full-time role, you'll work closely with families and Child Protection, ensuring safety and stability. Ideal candidates have a relevant qualification and experience working with at-risk youth, along with excellent communication and relationship-building skills.
Step into a rewarding role supporting children and young people in foster and kinship care across the Wimmera. You’ll provide high‑quality trauma‑informed case management focused on safety, stability and well‑being.
You will work closely with carers, families, Child Protection and allied professionals to develop individualised plans, monitor progress and advocate for children’s needs. Across your caseload you will contribute to permanency planning, coordinate care‑team meetings and ensure the voices of children and young people inform decisions that affect them.
You’ll also provide guidance and mentoring to colleagues and carers, support the referral and recruitment of new foster / kinship carers and be part of the after‑hours on‑call roster.
A relocation package of up to $5,000 can be negotiated to cover your move and assist with housing if you are considering relocating to Horsham.
You are experienced in case management and passionate about supporting children, young people and carers. You bring a child‑centred trauma‑informed and culturally respectful approach to your practice.
