A healthcare provider in the Basildon area is looking for a Healthy Family Support Practitioner to work with children aged 5-9 years and their families. As part of a dynamic team, you will provide essential support, empower parents, and work closely with various health and community services. Healthy Family Support Practitioner Full-Time vacancy working with children and or Young People 5- 9 years old and their families: Basildon Area.
We are seeking a passionate, dynamic and experienced 5+ yrs practitioner to join our Healthy Family Team. Our teams are fully integrated consisting of Health Visitors, School Nurses, Support Practitioners, Family Hub Practitioners and Community Connectors.

We work in partnershipwith childrens social care and health partners enabling the delivery ofintegrated health and social care services in a number of areas. We recognisethat to really transform childrens life chances, they need sustainedinterventions that meet the diversity of their needs and that participation andco-production must a key component of any service. If you enjoy pushing theboundaries of what works: working closely with other agencies to innovate andcreate; have the clinical expertise to lead on this project and want to workwith children and young people and their families to deliver service that trulymeet their needs, then we would like to hear from you.
Job Purpose
To provide advice, support and services to children, young people and their families in accordance with the aims and objectives of the service. Specifics of Service attached as applicable.
Key Responsibilities
For Service Users
Under guidance and instruction
To work with children, young people and their families, on a one to one or group basis, using a variety of interventions, to identify needs and to achieve positive outcomes.
To work in partnership with parents/carers to empower them and to enable them to achieve better outcomes for their children.
To work co-operatively with children's services, health agencies and other community based services, to provide the most effective service for children young people and families.
To apply safeguarding and child protection procedures.
