A community care organization in Sunderland is seeking a dedicated Healthcare Assessor to develop personalized care plans and ensure quality standards are met. The ideal candidate will have community care experience, superb communication skills, and a passion for putting people first. This full-time role offers competitive compensation, on-call payments, and additional benefits including 25 days holiday and an Employee Assistance Program.
