A healthcare organization is seeking a dedicated Health Visitor to work with the County Durham 0-5 Team. Located in Bishop Auckland or Ferryhill, the successful candidate will be responsible for promoting positive health care and delivering programs in collaboration with families and multi-disciplinary teams. This part-time role offers a salary range of £38,682 to £46,580 per annum, based on experience.

The Health Visitors role is to prevent ill health and promote positive holistic health care for an identified population and community and to take responsibility for ensuring the physical and mental health needs of families, acting as an advocate for the client and community.
Health Visitors plan, organise and deliver programmes of care in conjunction with the family and the multi-disciplinary team and undertake continuous ongoing assessment of children, young people and families. They provide an evidence based service to children, people, families and communities in accordance with National and Local Policy initiatives.
The role is underpinned by the principles of health visiting practice.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
