Job description
Overview

We are recruiting for a Band 4 Health Navigator as part of our Health and Justice team on a fixed term basis.

The post holder will be responsible for working within a multi-disciplinary team, with the flexibility of working across Sussex, dependent on skill and service need. The post holder will be part of a multi-disciplinary team, primarily completing screenings of individuals needs at different stages of their journey through the Criminal Justice System.

Responsibilities
  • Use screening tools to identify health and social care needs.
  • Assist individuals to navigate health and social care services to address identified needs.
  • Complete onward referrals where appropriate and provide short term intensive support to patients and their families.
  • Complete Court Reports outlining individuals nails of need that may be taken into account when decisions about charging and sentencing are made.
  • Work in a trauma informed way, providing appropriate liaison to prevent an individual having to "re-tell" their story.
  • Provide high quality information to key decision makers in youth and criminal justice agencies, including the police, courts, probation and Youth Offending Teams (YOTs).
  • Note: additional vetting from Sussex Police / His Majesty's Prison Service is required for roles based within Police Custody settings and Prison Settings.
