Job summary

A local government authority in Grays is seeking a Health & Care Improvement Officer to lead the development and implementation of Adult Social Care transformation programmes. Description

Job Title: Health & Care Improvement Officer

Salary: 45624 - 52038 per annum

Grade: F

Contract: Fixed Term Contract (2 years)

Full-Time: 37 Hours per Week

The role is responsible for the development and implementation of the Adult Social Care transformation programme - as part of Better Care Together Thurrock and the Integrated Care Strategy.

The role sits within Adult Social Care - but working in partnership with Housing and local NHS and community partners.

Experience and Skills
  • A good understanding of health and social care.
  • A good understanding of communities and voluntary sector agencies.
  • Experience of delivering change initiatives.
  • The ability to work in partnership with others.
  • A Standard Disclosure and Barring Service (DBS) check is required.
