Job summary

A position has arisen for the post of Health Care Support Worker within Mold Community Hospital. The post holder will be required to rotate within departments and onto night duty as required.

The team consists of nursing staff, HCAs, domestics and administrative staff who work together to provide high levels of patient care. The team are flexible in their approach to all aspects of patient care, always putting patient needs first.

Job responsibilities
  • Assist the registered nurse to deliver a high standard of holistic patient care, ensuring patients’ dignity and equality are maintained at all times.
  • Ensure the principles set out in the fundamentals of care are incorporated into daily practice to provide high quality service for patients and clients.
  • Assist patients to maintain their own hygiene and personal appearance.
  • Perform duties within the plan of care delegated by a qualified practitioner, only for which the post holder has relevant training and assessment.
  • Prepare ward/clinical area for receiving patients, ensure areas are clean and tidy, assist in maintaining standards and report untidiness to the appropriate person.
  • Assist in the admission of new patients, prepare bed area, welcome patient, direct to bed/area, and show patient around the ward.
  • Complete patient menu cards involving patients, considering any dietary instructions identified by the qualified nurse.
  • Participate in protected mealtimes, ensuring patients receive meals/beverages in a timely and safe manner, and maintain a conducive mealtime environment.
  • Assist with serving meals, open packets, help patients sit up, and support nutritional needs as directed; report any poor dietary intake to the nurse in charge.
  • Act as a chaperone for patients undergoing examinations and accompany patients to X-ray or other departments as directed by a registered nurse.
  • Maintain privacy and dignity during personal hygiene and toileting for patients who are unable to care for themselves.
  • Perform patient observations (temperature, pulse, respirations) following appropriate training and assessment.
  • Accurately document information relating to patient care as instructed by the nurse in charge.
  • Observe and report changes in patients’ conditions to the Registered Nurse.
  • Provide psychological and physical support to patients/clients before, during and after procedures or investigations.
  • Assist in moving and handling of patients safely in line with organisational guidelines and policies.
  • Facilitate effective communication between patients and carers/relatives.
  • Recognise and respond appropriately to urgent and emergency situations.
  • Communicate basic information to patients and carers with confidentiality, dignity and respect.
  • Undertake last offices and provide support to family and/or friends.
Person specification

Qualifications and/or Knowledge

  • Essential: Basic numeracy and literacy skills; agreement to complete All Wales Induction Framework Level 2 in health and social care or equivalent.
  • Desirable: Completion of All Wales Induction Framework.

Experience

  • Essential: Experience to fulfil the requirements of the post; experience of dealing with the public.
  • Desirable: Previous clinical experience.

Aptitude and Abilities

  • Essential: Ability to remain calm and professional in challenging circumstances.
  • Desirable: Ability to speak Welsh.
