A healthcare organization in Wooler seeks a Health and Wellbeing Coach to empower patients in managing their health through coaching techniques. You will support individuals and groups, fostering self-management and lifestyle changes. Ideal candidates have a relevant degree, are skilled in communication, and possess experience in health-related coaching. A driving license is required. Health and Wellbeing Coaches support patients to improve and their physical and mental health and wellbeing. It is a non-clinical role, using coaching and behaviour change skills to empower patients to manage, improve and prevent health issues. Coaches support people to think through their own challenges and goals, increasing their self-efficacy, motivation and commitment to move forwards to make changes to their lifestyle and improve their health.
The post holder will work within the existing Well Up North Health and Wellbeing Coaching Team, collaborating with clinical teams within GP practices in the Primary Care Network.
The role involves working with patients on both a 1 to 1 and group basis to provide personalised behaviour and lifestyle change support as well as referring to support services where appropriate. A full list of responsibilities can be found in the job description.
The post holder will be working as part of the Well Up North Primary Care Network (PCN). We have a combined list of over 62,000 patients across a group of 7 practices: Well Close Medical Group, Union Brae and Norham Practice, Wooler Health, Belford Medical Practice, Alnwick Medical Group, Greystoke Surgery, Gas House Lane Surgery.
Provide behaviour and lifestyle change support to patients across a wide range of physical and low level mental health conditions in both individual and group settings.
Promote self-management and independence by providing personalised support that enables individuals to take control of their health, make informed decisions, and improve health and wellbeing outcomes.
Signpost and refer patients to relevant services for further support where appropriate.
Complete a Personalised Care Institue (PCI) accredited coaching skills training course, where necessary, and engage in ongoing professional development including regular coaching supervision.
Stay up to date with local and national health issues, services and resources.
Collaborate with other teams and staff members to identify and enhance the scope of the role in line with evolving patient, practice and PCN needs.
Actively promote the Health and Wellbeing Coaching service among patients and staff, seeking opportunities to support practices and patient care within the scope of the role.
Contribute to the evaluation of the impact of Health and Wellbeing Coaching.
