Head of Integrated Care PCN - Northern | 9578 | Permanent Contract | 37 hours per week | £66,140 to £73,800 per annum | (Grade N) | North Walsham

This is your chance to take a key leadership role in embedding Human Learning Systems principles, fostering genuine collaboration and community-led support. You will work alongside healthcare leaders, social care professionals, commissioners, and third sector partners to create joined-up services that put people's strengths, assets, and aspirations first.
You will be responsible for leading adult social care teams in north Norfolk; strengthening integration across health and care, building productive networks, and delivering high-quality, sustainable services. You will empower staff, drive service innovation, and ensure that our services meet the evolving needs of Norfolk's diverse communities. You will co-produce solutions with residents, embed national and local initiatives, and help shape the future of preventative, relational care.
