A local healthcare provider seeks a Practice Nurse in Truro. Responsibilities include assessing and treating patients, conducting chronic disease management, and participating in vaccinations. Candidates must have RGN qualifications and experience within a GP practice. This is a full-time role with core duties from Monday to Friday between 8:00 AM to 6:00 PM.
We are looking for a Practice Nurse to join our nursing team.
Ideally you will work between 32 hours a week, between the hours of 8:00am and 6:00pm, Monday to Friday.
The post holder will be responsible for the care and management of delivering nursing services to our patients.
Probus Surgery is a well-established Primary Care business offering NHS services to our local population, with a population of circa 9500 patients. Probus is our main surgery, with branch sites at The Merlin Centre and Tregony. Our clinical team consists of GPs, Advanced Clinical Practitioners, Nurse Practitioners, Paramedics, Nurses, HCAs, Pharmacists, Dispensers, Care Co-Ordinators, Social Prescribers, Receptionists and Admin.
The following are the core responsibilities of a PracticeNurse. There may be occasions when there is a requirement to carry outother tasks; this will be dependent on factors such as workload, staffinglevels and direction of the Clinical Team or Senior Manager:
Independently assess, diagnose, treat, refer, signpost and dischargepatients and service users as necessary and in accordance with clinical basedevidence and NICE guidelines
Undertake chronic disease reviews, including diabetes, COPD, asthma, heartfailure, learning disabilities, etc.
Undertake telephone triage
Undertake the collection of pathology specimens, includingblood samples, swabs, etc.
Undertake simple wound and complex dressings, inclusive ofleg ulcers
Undertake smearsUndertake contraception/pill checks and support our womanhealth clinics
Perform investigatory procedures as requested by colleagues
Use appropriate infection control procedures
Be aware of safe storage of equipment, and drugs
Ensure clinical treatment is safe, effective and conductedwithin the boundaries of competence, and to acknowledge limitationsEnsure personal and professional clinical standards aremaintained
Adapt styles of communication in order to empowerself-management and health education for patients
Participate in vaccination programmes, vaccinating patientsin the Practice, and/or on home visits
Formally and informally, impart knowledge and skills tocolleagues, promoting peer review and best practice within the workingenvironment
Communicate at all levels within the team, ensuring aneffective service is delivered
Work within Practice policies and procedures
Enhance own performance through continuous professionaldevelopment, imparting own knowledge and behaviours to meet the needs of theservice
Contribute positively to the effectiveness and efficiency ofthe team, and work colleagues
Establish and maintain good liaison with other Practices, andthe wider system, including secondary careSupport the clinical team with safeguarding matters, inaccordance with local and national policies and refer safeguarding concerns,using established safeguarding referral pathways
Delegate tasks clearly and appropriately, adopting theprincipals of safe practice and assessment of competence of those colleaguestaking on the delegated duty
Ensure accurate and legible notes, of all consultations andtreatment, are recorded in the patients notes
Ensure the clinical system is kept up to date with accuratecoding, using templates where appropriate
Ensure the appropriate items of service claims are madeaccurately, reporting issues to the relevant person
Collect and maintain statistical information, required forregular and ad hoc reports and audits
Attend and participate in Practice meetings, as requiredUndertake any other additional duties appropriate to the postas requested by the Partners or Managers
