Managing own caseload
Developing and auditing standardsof nursing care
Running Chronic Disease Clinics,call & recall of patients and case management of these patients
Providing family planning servicesincluding smears
PreventativeHealth and Chronic Disease Management
Managing and evaluating screeningand immunisation programmes
Providing health screening, plannedand ad hoc, to patients of all ages and advising & taking action onthe findings
Managing the nursing care of allthose patients with chronic disease and maintaining an appointment andrecall system for patients
Assisting with the implementation,review and maintenance of infection control measures in the Practice
Development,Teaching & Education
Participatingin the continuing identification and assessment of the care needs of thepractice population
Forming ownPersonal Development Plan
Participatingin the process of clinical supervision
Organisation/Administration
Initiating/contributingto the development and review of practice protocols
Maintainingequipment and supplies and ensuring correct storage of medical and surgicalequipment, including vaccines
Liaisingwith members of the wider Primary Health Care Team and agencies outside thepractice to ensure appropriate care is provided

To provide andmaintain a high standard of nursing care for patients as well as providingnursing assistance to the GPs and other members of the primary care team.
An experiencedpractice nurse, the post holder will be a key player in delivering the primarycare agenda.
Clinical
Providing nursing care and clinicalservices for a full range of procedures such as vaccinations/immunisation.
Managing own caseload

Participatingas an active member of the Primary Health Care Team and attending meetingsregularly
Meetings
It will be necessary to attend andcontribute to various practice meetings as requested. The only reason fornot attending will be annual, study or sick leave.
