Overview

Sunnybank Health Centre is a GP practice serving the Cefn Fforest/Blackwood area. We have a list size of around 5700 patients. The practice delivers a high standard of patient care and offers a supportive and flexible working environment. Our team includes GPs, Minor Illness & Practice Nurse, HCSW, Independent Prescribing Clinical Pharmacist, a well organised administration team and allied healthcare professionals.

Job responsibilities

• The successful candidate will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
• In accordance with job plan, undertake duties including surgery consultations, telephone consultations and queries, visiting patients at home, reviewing pathology results, checking and signing repeat prescriptions, and dealing with paperwork and correspondence in a timely fashion.
• Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
• Screen patients for disease risk factors and early signs of illness.
• Develop care plans for health in consultation with patients and in line with current practice disease management protocols.
• Provide health education and record clear and contemporaneous consultation notes to agreed standards.
• Prescribe in accordance with the practice prescribing formulary (or generically) when clinically appropriate.

Essential qualifications and requirements

• Fully qualified GP with GMC registration.
• Annual appraisal and revalidation (when appropriate).
• General practice (Vocational Training Scheme) trained.

• UK driving licence.
