A regional healthcare organization in England is seeking a Frailty Team Administrator to provide high-quality administrative support within the Primary Care Network. The role involves managing clinical data, coordinating meetings, and supporting multidisciplinary teams to enhance patient care for those living with frailty. Ideal candidates should possess strong IT skills, a background in administration, and excellent verbal and written communication abilities.
We are looking for a Frailty Team Administrator to provide high-quality administrative support to the Frailty Team within the Primary Care Network (PCN), contributing to the delivery of proactive, coordinated care for patients living with frailty.
The role involves supporting multidisciplinary working, managing clinical and non-clinical data, coordinating meetings, liaising with patients and external partners, and ensuring smooth operational processes within the frailty pathway. You will be a key point of contact for the team and help to ensure efficient day-to-day administration. The role supports the lead frailty nurse, acute visiting service, and works alongside care coordinators to aid with supporting safe patient care. You will be required to use a range of computer systems. As a first point of contact for patients, you will identify and prioritise information to inform decisions on the urgency of a response. The ability to work on your own initiative, prioritise workload, and meet deadlines is essential.
