A healthcare provider in Christchurch is looking for an enthusiastic Frailty Nurse to deliver high-quality care to frail patients in their community. The successful candidate will collaborate with GPs and receive support from various healthcare professionals. Applicants must have GCSEs in English and Maths and a valid driving license. The successfulcandidate will be driven and highly motivated with good organisational andinterpersonal skills. You will be flexible, thorough, and passionate aboutdelivering high quality care to our Frail patients in the community.
Working under the guidance of the GP, the FrailtyNurse will receive daily support from Frailty Care Coordinators, ClinicalPharmacists, and the wider PCN.
Please note, a valid driving licence and access toyour own vehicle are essential for this role.
Undertake clinical nursing practiceusing expert knowledge and clinical skills to deliver holistic care to patientsliving in their own home or care home.
Collaborate closely with GPs and workindependently to deliver safe, effective care to frail and houseboundindividuals.
Facilitate patient education,self-management of disease, and behaviour modification.
Conduct thorough person-centred needsassessment to develop care plans.
Initiate the process of diagnosiswith patients suspected to have a chronic disease.
Participate in team meetings, audits,and data collection for improving patient care.
Support HCAs / Care Coordinators indelivering high-quality care to frail and housebound patients.
Provide expert clinical advice andsupport to patients, carers, and colleagues, maintaining high standards ofprofessional practice and clinical excellence.
Operate in line with clinicalprotocols and guidelines, ensuring professional, compassionate support forpatients, families, and carers - using resources responsibly and efficiently.
Ensure compliance with practice CQCrequirements and maintain accurate documentation.
Work within all relevant practicepolicies and procedural guidelines e.g., infection control, chaperoning, riskmanagement.

Contribute to disease registers.
Keep up to date with schemes andcontractual agreements by liaising with Practice Managers, GPs and IntegratedCare Board.
