Overview

Experienced Social Worker Position Within Our Home First Team located in Sherborne, North Dorset. Home First is a partnership approach to provide more community-based care in Dorset and reduce unnecessary hospital stays. We promote a discharge to assess model of working with adults who have been discharged from hospital either to their own home or to an assessment bed, working collaboratively with and alongside our health colleagues and other partners to achieve this.

What you can expect to be doing

You will be undertaking strengths‑based assessments, support planning and reviews to support customers to achieve their outcomes and promote their independence. You will be working as part of a busy team with multidisciplinary practice, being and working collaboratively with our health colleagues and other professionals integral to the role. You will complete Mental Capacity Assessments and Court of Protection.

  • Able to work and contribute to a team.
  • Excellent innovation skills with a flexible approach when needed.
  • Excellent communication skills, enjoy working in a fast‑paced environment, have a flexible approach and be able to work closely with a range of professionals.
  • Confident with the legal basis for practice and demonstrate a good understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998 and Equality Act 2010.

  • A degree or equivalent in social work and be registered with Social Work England, with ability to fulfil travel requirements.
