Role Overview

Are you passionate about helping people regain independence? Join our ERS@Home team as a Care Assessor/Reviewer and play a vital role in delivering high-quality reablement care across Hampshire and the Isle of Wight. The team supports people in the community to prevent admission to hospital as well as when they return home from hospital.

You'll assess and review care plans, collaborate with therapy teams, and support patients in their recovery journey.

  • Maintain accurate and timely documentation
  • Identify and escalated clinical risks
  • Support service development and quality improvement
Qualifications
