Doncaster Strategic Transfer of Care Hub Manager – Doncaster, Rotherham, and South Humber NHSFT. Closing date: 15 October 2025.
Working across the health and social care landscape the post holder will lead, coordinate and provide forward momentum to the programme of change related to the Transfer of Care Hub, based at Doncaster and Bassetlaw Teaching Hospital. The role sits across South Yorkshire Integrated Care Board (SY ICB), Rotherham, Doncaster and South Humber NHS Foundation Trust (RDaSH), City of Doncaster Council and Doncaster and Bassetlaw Teaching Hospitals.
The Care Transfer Hub is centred around Home First, why not home, why not today. We want to reduce the risk of harm to the person by delays in their transfer of care and we want to improve people\'s outcomes, home is best. The Strategic Transfer of care Hub Manager will provide operational leadership and coordination to enable transfers of care from both acute and community physical health beds and support recovery in the community. This includes the establishment of appropriate governance and assurance, monitoring progress, managing risks and issues and ensuring business readiness for change.
