Therehas never been a more exciting time to work in primary care, and The WatersidePrimary Care Network (PCN) is expanding its Clinical Team.

Weare seeking to recruit a positive, caring, and self-motivated individual totake on the role of District Nurse within our new Proactive Care Team. Thisnew team provides a dedicated multidisciplinary team, allowing us to deliverstandardised, high-quality care for care home residents and other houseboundpatients, thereby reducing inefficiencies and enhancing proactive healthcareservices. This approach aligns with our vision of proactive, patient-centredcare.

TheWaterside PCN are looking for an enthusiastic District Nurse to join our teamon a fixed term basis of 2 years. The role will be part of the GP ResilienceScheme (Enhanced Service) and may have the potential to be extended beyond 2years however this is subject to NHS funding and guidance.

Therole will be based across several sites: our local care homes and individualresidences across the Waterside area, our three Practices within the PCN and inour Urgent Care Centre at Hythe Hospital.

We are flexible with the number of sessions and would welcome allapplications.

Main duties of the job

The District Nurse is responsible for providing clinical nursing careincluding wound management, chronic disease monitoring and palliative caresupport to our housebound patients, both in local care homes and in personalresidences.

They will be an integral part of and support Proactive Care Team andwill be involved with designing, implementing and evaluating programmes of carefor this vulnerable group of patients, including preventative deterioration ofcomplex health, social prescribing and signposting, management of comorbiditiesand supporting the prevention of an urgent care need.

Job responsibilities

You will take responsibility and professionalaccountability for the management of a defined caseload of housebound patientsincluding the delegation of tasks to other members of the Proactive Care Team.

Prescribe within your individual scope of practiceand ensure that nurse prescribing is undertaken in a safe, cost-effectivemanner.

Develop a caseload profile, identifying thosegroups with specific health needs and implement and evaluate programmes of careto target groups taking a case management approach.

Act in accordance with current SafeguardingVulnerable Adults and Trust guidelines/policy to protect vulnerable adults.

Provide appropriate nursing care to patients on yourdefined caseload. Provide specialist advice appropriate to health and socialcare to patients, carers, relatives and other agencies.

To attend the Proactive Care Team meetings, reviewingpatients, contributing ideas and developing strategies for housebound patients.

Demonstrate excellent communication and negotiationskills in situations which may be complex, highly sensitive and emotive. Allcommunication will be underpinned by the principles of data protection, dignityand confidentiality.

Take responsibility for initiating and/or engagingin open discussion with statutory and voluntary agencies and all members of thewider multi-disciplinary team as appropriate.

Ensure effective communication to supportcontinuity of care on transfer between hospital, community, residential andcare home settings and vice versa.

Promotes and maintains accurate record keeping,confirming to policy and data protection legislation.

Responsible for communicating and receivingsensitive, complex, and sometimes contentious information, dealing effectivelywith staff, relatives, and carers whilst ensuring confidentiality. Will usedeveloped persuasive, motivational, negotiating skills, where agreement andcooperation is required.

To input information by electronically recordingdaily activity on EMIS, ensuring each contact/activity is recorded in realtime.

Demonstrate and contribute to a positive teamculture which promotes a collaborative and supportive environment. Supportingthe Lead GP of the team and being receptive to mentorship, clinicalsupervision, openness and candour, and provision of an educational environment.

Take responsibility for the complex and holisticassessment or delegation of planning, implementation and evaluation of care onan individual basis, utilising recognised models and tools, e.g. end of lifecare and long-term conditions.

Diagnostic assessment, routinely utilising patienthead to toe examinations in order to diagnose, implement treatment plan,including prescribing, and/or escalating when clinically required andappropriate.

Be prepared to be flexible and to provide cover asrequired in our Urgent Care Centre or for our reactive home visiting team asdemand requires, e.g. in response to increased winter pressures.

As a District Nurse there will be a need for a varied clinical skillset, including, but not limited to the following -

End of Life Care - symptom control

Wound Care Including complex e.g. Vacuum assistedtherapy

Leg Ulcers Including compressions therapy

Patient education and training of self-managementtechniques

Multi Comorbidities and prevention of deteriorationmanagement e.g. Heart Failure, Diabetes, Respiratory, Frailty, Dementia

Social prescribing

Be proactive in offering health promotion andparticipate in health promotion activities for a defined caseload.

You will be able to quickly build excellent working relationships andcommunication links with all stakeholders including Local Authorities, SocialCare teams and voluntary services as appropriate for patient care.
