A leading healthcare provider in Stoke on Trent is seeking a Discharge Facilitator to support safe and timely patient discharge across hospital and community settings. The ideal candidate will have strong organizational and communication skills, along with experience in healthcare. This role offers a salary between £24,937 to £26,598 per year with a permanent, full-time contract.

We are offering an exciting opportunity to join Discharge to Access (D2A) Complex Assessment team as a Discharge Facilitator based in Stoke on Trent.
You will have responsibility for assessing and tracking patients requiring a discharge-to-assess (D2A) bed-based service within a timely manner in a non-acute setting and closely work with the multi-disciplinary team and external agencies to ensure a robust assessment and a safe discharge.
You will work alongside the Patient Flow Team in the D2A and community hospital settings to facilitate an efficient, effective and timely utilisation of beds and resources. You will be required to effectively provide support to assist in the delivery of safe and effective patient transfer and tracking within D2A Services whilst gathering and collating collateral histories and information regarding current care needs.
The service operates 24/7, 365 days per year and you will be expected to work on a 5 day per week shift pattern. This may include some weekends and bank holidays.
As Discharge Facilitator you will be responsible for the clerical coordination of the discharge process from the point of admission until discharge whilst building robust relationships with patient's families and the multi-disciplinary team.
