There is an exciting opportunity to work withinthe Specialty and Integrated Medicine CSU as a Discharge Coordinator. Thisinvolves working between one or more clinical areas to facilitate patientdischarges, providing a high quality service in line with Trust standards. Theywill be able to communicate sensitively and effectively with a wide range ofpeople including patients, families, colleagues and agencies external to theTrust.
The discharge co-ordinator will facilitatetimely, safe, and effective patient discharge through close collaboration withmultidisciplinary teams and external agencies. Participate in board rounds andMDT meetings to ensure clear, accurate discharge plans documented in PPM+.Communicate sensitively with patients, families, and professionals to provideup-to-date information on discharge pathways. Monitor patient flow, identifydelays, and escalate issues promptly to support seamless transitions of care.Maintain accurate records of discharge status, waiting times, and reasons fordelay, ensuring compliance with Trust policies. Promote teamwork, guide lessexperienced colleagues, and uphold confidentiality, safeguarding, and health& safety standards while adapting flexibly to service demands
Weare a diverse team working within the Specialty and Integrated Medicine CSU.Our discharge coordinators provide discharge coverage for all inpatient wardswithin the CSU including Elderly medicine, General medicine, and InfectiousDiseases.
