Job description

We have an exciting opportunity for both full-time and part-time registered nurses to join a team of dedicated Diabetes Specialist Nurses in the Elsie Bertram Diabetes Centre, working across our 3 teams within the trust. At the EBDC, we have a firmly established multi-professional approach to care delivery with both Consultant and Specialist Nurses delivering patient clinics. We also work closely with GP surgeries within our Facilitator team in the community., As a Diabetes Specialist Nurse/Facilitator, you will be involved in, following appropriate training, covering inpatient services, outpatient services and community clinics, including our specialist outreach clinics in antenatal and young adult services. A flexible approach will be welcomed and may involve some later clinic cover and weekends. As part of your role, you will help to provide patient education on an individual basis, as well as part of a group setting, initiate medication including insulin/GLP-1 therapy and latest technology used in diabetes care such as insulin pumps and CGM for people who need them. The post will involve ongoing review and support of our patients, their carers and/or family members with management of their diabetes in our specialist nurse-led and multidisciplinary clinics. You will also be providing education to staff members of the wider MDT within the Trust. There will also be opportunities for further development and the potential to undertake Non-Medical Nurse Prescribing.
