Overview

The post holder will lead the nursing diabetes team, with enthusiasm and willingness to develop knowledge and skills, and a shared vision to be involved in service development projects and improve patient care. The post holder will have experience of clinical leadership, a determination to improve quality of patient‑centred care, service delivery and service team projects.

The post holder will work as an integral part of the established specialist Diabetes Nursing Team as the diabetes lead for Calderdale and Huddersfield NHS Trust.

Responsibilities
  • Lead the nursing diabetes team with enthusiasm to develop knowledge and skills and contribute to service development projects and improved patient care.
  • Demonstrate clinical leadership to improve the quality of patient‑centred care, service delivery and service team projects.
  • Manage and develop a team, including completing staff appraisals, sickness monitoring and roster management.
  • Possess advanced diabetes knowledge and experience in caring for people with Type 1 and Type 2 diabetes, in either an acute or primary care setting.
  • Deliver autonomous diabetes care at level 4 and level 5, including both inpatient and outpatient care.
  • Be self‑motivated, organised and adaptable to changing service needs; demonstrate excellent interpersonal skills; be computer literate; preferably familiar with SystmOne and EPR.
  • Work as part of the established specialist Diabetes Nursing Team as the diabetes lead for Calderdale and Huddersfield NHS Trust.
  • Engage with service development, improvement initiatives and learning opportunities to enhance patient care.
Qualifications/Experience
  • Advanced diabetes knowledge and experience looking after people with Type 1 and Type 2 diabetes, either in an acute or primary care setting.
