Job description
Overview

We have an exciting opportunity for a NMC Registered Band 6 Nurse to join our Diabetes Specialist Service based in South Staffordshire. You will join our team of Specialist Nurses, Consultants, Educators, and Administration staff. The aim of the service is to provide Specialist Diabetes support and interventions within the Community and across the primary care network through nurse led clinics and home visits.

Responsibilities
  • Provide assessment of complex/specialist care/support needs and in the development, implementation, and evaluation of care/support packages.
  • Provide supervisory, training and leadership responsibilities where appropriate and have some responsibility for team performance and service developments/improvements.
  • Demonstrate initiative and be creative in finding solutions to problems.
  • Work with the team leader undertaking delegated tasks in team management.
Qualifications and skills
  • Extensive and demonstrable skills in Diabetes management; ideally hold an accredited course in Diabetes or demonstrate a keen interest in Diabetes management with a focus on education, teaching and mentorship for health care professionals and patients.
  • Non-medical prescribing is advantageous though not essential.
  • Willingness and ability to travel independently across a large geographical area.
  • Detailed professional and clinical knowledge with focus in Diabetes care and management as a specialist practitioner.
