Job description
Overview

Bolton Hospital Foundation Trust are looking for a Diabetes and Endocrine Specialist Nurse, Band 7 for a full time post. This is a unique post where the successful candidate would work full time split over 2 specialities, working 2.5 days as a Diabetes specialist Nurse and 2.5 days as a Endocrine specialist nurse. Band 6 - 7 development would be considered for a suitable candidate.

Responsibilities
  • To work as an autonomous, advanced practitioner and effectively deliver an expert service within the specialist area from an inpatient and outpatient base.
  • To contribute to a high quality integrated Diabetes and Endocrine service that provides care for patients who have a diagnosis of Diabetes or established Endocrine conditions within primary, secondary and specialist care.
  • To provide specialist care to patients ensuring both the patient and their families are fully informed and supported.
  • To act as a resource and educator for both patients and staff in a hospital and community setting, providing advice and knowledge in managing these high-risk groups of patients.
  • Maintain working relationships with the Diabetes and Endocrine MDT including Consultants, Specialist Nurses, Podiatrists, Dietitians, Support staff and clerical teams.
