Job description

An exciting opportunity has arisen for a D2A Co-ordinator to join our team at Somerset NHS Foundation Trust.

The D2A Hub, based at County Hall in Taunton, provide comprehensive co-ordination and administration for the Discharge to assess / Intermediate care county wide discharge service.

We have 1 permanent position available.

Main duties of the job

You will join a team of 7 Coordinators who facilitate flow through the D2A service, ensuring, with support from clinical leads, maximum usage of capacity across the service.

You will help coordinate and run the daily Discharge to assess pathway HUB, recording activity on electronic and paper systems to monitor individual patient level information, and provide data analysis and reports in various formats as required.

You will also liaise with and coordinate D2A Keyworkers, providers, and referring organisations to coordinate the discharge of individual patients.

Please note it is shift working 8-8 Monday to Friday and 8-6 Saturday Sunday and bank holidays. Generally this is 1 weekend in 5 and it is 3 shifts a week, sometimes 4 if hours need to be made up across a month.

Experience
  • Knowledge of the wider effects of delayed discharges to our county services, our patients and our acute and community partners.
  • Experience in teaching/supervising others
  • Knowledge of discharge processes and policies.
