Hello, I’m Danny, and I lead the Calderdale Intensive Home-Based Treatment Team (IHBT). We are looking for a Band 6 Senior Practitioner to join our supportive and dedicated team, who are passionate about providing outstanding person-centred care to people when they need it most.

Our team offers an alternative to hospital admission by delivering intensive home-based treatment to acutely unwell adults, as well as coordinating assessments for people detained under section 136 MHA in the locality. As a Senior Practitioner, you will play a key role in assessment, risk management, care planning and coordination of care delivery. This role requires flexibility as we operate a 7-day service, including weekends and nights, and provides the opportunity to work across the health and social care spectrum alongside multi-disciplinary colleagues and third sector providers. Most importantly, you will bring compassion and commitment to engaging with people in their daily lives, supporting safe and timely recovery and making a real difference.
Applicants should be a Registered Mental Health Nurse with working age adult experience at Band 5 or above. A full driving licence and access to a car are essential.
