Job description
Overview

We are seeking to appoint an enthusiastic, motivated, forward thinking Consultant in Palliative Medicine to an innovative community post in Bolton. The post holder will be joining the service at a time of exciting change and development to improve the provision of palliative care to the residents of Bolton. The post is predominantly community-based, with integrated hospital-based services.

Responsibilities
  • Provide clinical leadership and service development support to set up and expand new services, including a 7 day rapid response service and a ‘Hospital at Home’ virtual ward delivered by senior clinical practitioners, alongside advanced clinical practitioners in the Admissions Avoidance Team and other specialist teams running virtual wards.
  • Work alongside a consultant colleague in the community setting to support newly appointed community senior clinical practitioners and a specialty doctor.
  • Engage, lead and drive changes to prove benefits and expand the team to support the service.
  • Participate in on-call arrangements as determined; weekend working to support the new service will be piloted, with current consultants providing 1:5 frequency 2nd on-call cover to Bolton Hospice.
  • Operate predominantly in the community, with integrated hospital-based elements as required.
  • Involvement in developing 7 day services and the integration of Hospital at Home/virtual ward initiatives.
Context and Service Model

The service includes a proactive emergency department service by two senior clinical practitioners (8a) to identify and manage unmet palliative care needs and initiate advance care planning, with a track record of reducing admissions and length of stay. The service aims to expand these capabilities into the community setting.

The post holder will be joining Bolton NHS Foundation Trust, an integrated provider serving Bolton’s population, with opportunities to work closely with primary care and local authority partners to deliver seamless patient care. The Trust strives to deliver high-quality care and staff development, and has been rated as Good by the Care Quality Commission (CQC).

Notes

There is currently no requirement to provide an outpatient clinic.
