A community support organization is looking for a Community Support Worker to join their Young People with Disabilities Team in Colchester. The ideal candidate will support young people and adults, ensuring their independence and wellbeing. Responsibilities include assessments and support planning, and collaboration with social workers. The Young People with Disabilities Service works with young people and adults from 16-25 years, through supporting and promoting their safety, independence, and wellbeing from childhood into adulthood. The team operate with a strengths‑focussed approach, in line with ECC's transformational shift from a focus on long‑term care support, to a model with four key principles: prevention; early intervention; enablement and safeguarding.
This will enable young people, their families and carers to access information and tools, live healthily and independently and for our function an opportunity to respond pro‑actively to growing service demand in a sustainable way, within the financial constraints of the Council.
This role will work in conjunction with Qualified Social Workers to deliver bespoke, targeted intervention work, in accordance with the care plan, to vulnerable adults and young people.
With relevant experience of activity based working with vulnerable adults and/or young people, in a social care setting, or educated to Level 3. You will be passionate about engaging with and providing direct support in the community, working with partners as required.
