A community healthcare provider in Tameside is seeking a Community Staff Nurse to join their District Nursing Service. Community Staff Nurse — Band 5. Permanent position with the Tameside & Glossop District Nursing Service. Service works across the 5 neighbourhoods of Tameside. You will be allocated a team base on appointment and will have support from an induction facilitator and a designated buddy. Newly qualified applicants will undertake a preceptorship programme. The post may require work across localities as a service need.

The post holder will work within a Multi-Disciplinary Team in the Community and, after a period of induction, will be responsible for the assessment of patient care needs and development, implementation and evaluation of programmes of care for a specific group of clients as the named care provider, and in supporting the care of all client groups within the community. You will act in accordance with locally agreed standards of care and ensure that clinical procedures are correctly carried out. As an experienced nurse, and as appropriate, you will coordinate the caseload in the absence of the team/senior lead, ensuring effective communication at all times, and providing support for junior staff within the team. You will play a key role in the supervision and assessment of pre- and post-basic learners and unregistered staff, providing a teaching role and acting as named supervisor.
