Job description
Overview

As a Community Nurse working in the Community Response Team (CRT) you will assess, identify and manage the needs of patients who have experienced a deterioration in health. You will provide holistic and comprehensive care to patients in their own homes, support patients at the end of life, and work to prevent hospital admissions and facilitate timely discharges.

Key Responsibilities
  • Conduct first contact assessments and develop, implement, evaluate and reassess care plans.
  • Manage caseload, prioritise care and delegate workload appropriately to team members.
  • Act as a patient advocate and promote accurate record‑keeping in line with NMC policy and data protection.
  • Communicate effectively with service users, health‑care professionals and statutory and voluntary organisations.
  • Plan and deliver planned and opportunistic health promotion and education activities with patients and carers.
  • Work collaboratively with the multidisciplinary team, including therapists, GPs, social services and urgent response teams.
  • Participate in mandatory and statutory training, maintenance of professional competency and continuous improvement.
Qualifications & Skills
  • NMC registration and compliance with the NMC Code of Professional Conduct.
  • Valid driving licence and access to a personal car for clinical travel.
  • Experience in community nursing, including assessment, care planning and multidisciplinary collaboration.
  • Strong communication and interpersonal skills, with the ability to advocate for patients and maintain accurate clinical records.
  • Organisational and time‑management skills to manage an unpredictable and unscheduled workload.
