Position Overview

Community Staff Nurse in the Healthy Living Service within the Community Health Team at Suffolk County Council.

  • Location: Wellington Children's Centre, Ipswich IP1 2PB (Community based)
  • Salary: £35,412 – £40,777 per annum pro-rata
  • Working hours: 18.5 hours per week
  • Contract: Permanent
Responsibilities
  • clinically review referrals into the Healthy Living Service and take appropriate actions
  • undertake clinical and wellbeing supervisions for the Healthy Living Practitioners
  • support daily operational components of the Healthy Living Service
  • work in accordance with the NMC Code of Professional Conduct and maintain NMC registration
  • support children, young people and families through the care pathways outlined in the Healthy Living Service SOP, ensuring compliance with clinical governance frameworks and policies.
Qualifications & skills
  • NMC Registered Nurse
  • Ability to provide daily oversight and supervision of people and services
  • Knowledge of Quality Assurance and Performance Management
  • Skill to review child records for unhealthy weight, identify early interventions, and reduce risks
  • Understanding of information and advice to signpost children, young people and families to relevant services (local and beyond)
  • Ability to safeguard children, young people and vulnerable adults through effective practice and multi‑agency working, including participation in safeguarding meetings and assessments
  • Capacity to support children, young people and families in maintaining emotional health and wellbeing, helping them find support networks across voluntary, faith and social enterprise sectors

The Healthy Living Service is part of the Community Health Team within the Children and Young Peoples Service. It brings together health, social care and children’s centre professionals to support families during crucial years of a child’s life, offering tailored advice, assessments and interventions.
