A community healthcare provider in Dukinfield is seeking an experienced nurse to work within a Multi-Disciplinary Team. The role involves coordinating patient care, providing support for junior staff, and teaching. This position offers flexible working, 27-33 days of annual leave, and various benefits, making it ideal for a self-motivated individual passionate about community care.
The post holder will work within a Multi-Disciplinary Team in the Community and after period of induction will be responsible for the assessment of patient care needs and development, implementation and evaluation of programmes of care, both for a specific group of clients as the named care provider and in supporting the care of all client groups within the community.
As an experienced nurse and as appropriate, you will co-ordinate the case load in the absence of the team/senior lead ensuring that effective communication is maintained at all times, providing support for junior staff within the team.
You will pay a key role in the supervision and assessment of pre and post basic learners and unregistered staff providing a teaching role and acting as named supervisor.
