The role includes identifying care needs, monitoring patients' conditions, recording observations and helping them develop their wellbeing care plans.

You'd be supporting people to maintain their wellbeing and independence, helping to avoid unnecessary admission to hospital as well as delivering exceptional end of life care including symptom management and help in identifying and achieving people's preferences.
