A leading health organization in North Wales is seeking a passionate Band 5 Community Nurse to join their District Nursing Service. The role involves delivering high-quality evidence-based care to patients within their homes, enabling self-management and preventing hospital admissions. The ideal candidate will be a registered nurse with excellent communication skills, eager to develop within the community nursing field. Competitive salary offered between £31,516 to £38,364 per annum, pro rata.

The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Anglesey and Gwynedd working in an integrated way with partner organisations to deliver high quality evidence based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self management and implementing seamless patient centred care and treatment.
You must be able to travel between patients homes both within towns and rural areas in a timely manner.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
