Job description

We are offering an opportunity for Band 5 NMC Registered Staff Nurses to join our Community nursing teams in North Staffordshire. We have full time roles currently available in our:

  • Biddulph & Kidsgrove PCN team based at Kidsgrove Health Centre
  • Meir PCN team based in Longton Health Centre

We are looking for nurses who are committed to providing high standards of holistic care to join our busy, supportive & friendly teams. This is an excellent opportunity for a highly skilled nurse looking for a new challenge or looking to return to Community Nursing. Many people would be unable to live at home without the support they receive from the Community Nursing teams and community services are admitting patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Our Community Nursing teams assess, plan and deliver nursing care to patients within their own homes, in line with evidence-based practice. It is a privilege to care for patients in their own homes, and by joining our team you will be able to experience the difference this makes to our families within the community.

Key Responsibilities

This role includes daytime working, including weekends and cover of early morning shifts starting from 7am when required.

  • Work in collaboration with the muti-disciplinary team and colleagues to promote independence and support patient‑centred care planning.
  • Provide care directly to patients on the caseload with a range of nursing needs (palliative & end of life care, long term condition management, medication administration, complex wound care and bladder & bowel management).
  • Work autonomously, in line with your scope of practice, competence and policies.
  • Handover patient information to the wider team and document patient information effectively.
  • Escalate patients’ needs as appropriate.

Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.
