Job description
Overview

Care closer to home is becoming integral to the future of the NHS. The Community Health Services Division of NottsHC is leading this journey, improving physical health care services in local communities throughout Nottinghamshire. This post covers the Nottingham North and East or Rushcliffe area of South Nottinghamshire.

Role

You will work within the Discharge to Assess (Home First) service, providing community rehabilitation and reablement support to patients to maintain their independence in their own homes.

Responsibilities
  • Work under the direction of a registered professional as part of a multidisciplinary clinical team.
  • Deliver patient‑centred care by providing reablement, rehabilitation, and support programmes tailored to individual needs.
  • Assist patients with activities of daily living to enable them to remain in their community.
  • Support early discharge from hospital and prevent early readmission or admission to care.
  • Collaborate within the team to analyse and develop treatment plans and provide compassionate and timely support.
  • Maintain accurate and timely documentation of care activity.
Qualifications & Experience
  • Experience in community rehabilitation, reablement or similar supportive care roles.
  • Excellent communication, interpersonal and organisational skills.
  • Ability to work flexibly, reliably, resourcefully and adaptively in a range of settings.
  • Evidence of high levels of personal and clinical credibility.
Driving & Licensing

A full UK driving licence and vehicle for business use are required. Reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Working Hours

The service operates 7 days: 07:00‑22:00hrs.

Additional Information
  • You will be required to pay for a DBS check (cost £26.40 standard or £54.40 enhanced) deducted from salary over the first two months.
  • Enrolment in the DBS Update Service is encouraged; an annual fee of £16 applies.
  • Applicants requiring a Skilled Worker visa will need a role at Band 3 or above and not all Band 3 roles are sponsorable. For more information consult the Immigration Rules appendix.
