A leading community health service provider in Camberley is seeking a Community Rehab Assistant to deliver care and rehab in patients' homes. Working closely with ourIntermediate Care Team, as a Community Rehab Assistant you will support and assist the senior team members toprovide care and rehab to people in their own homes .Our work is crucial to the community as we aim to prevent unnecessary hospital submission. In this role, you will care for patients in the way thatyou believe is best, delivering our high service standards, sharing bestpractice, and actively improving the way we work.
As part of our community nursing team we empower our staff to work autonomously and with flexibility by providing them with state of the artnew tabletsfor them to be able to check in from home, write their notes up on the go and allow them to not be trapped in an office base, so they can really enjoy working in their chosen community background!
We cover a 7-day service. We share out the shifts and cover the service from 8am - 8pm. Having a driving licence and access to a vehicle to work in the community is essential for this role.
