We are seeking people who are adaptable and collaborative, with the potential to grow, and the ability to plan, prioritise and organise their work, to join our neighbourhood nursing service.

Our community nursing service which includes district nursing, continence care, neighbourhood nurse practitioners, end of life carers and phlebotomy organise, deliver and manage nursing care for patients in their own homes, residential accommodation or clinic settings. We also attend multi-disciplinary team meetings, promote/preserve the health needs of individuals, families and communities, encourage self-care as well as undertake assessment and evaluation of care.

The service operates a 7am to 7pm service, 365 days in the year.

The community nursing teams provide nursing care for patients, aged 18 and over who are housebound within the Medway area, delivering a range of personalised care and treatment to those with complex and long-term conditions, supporting people to stay safe and well at home.

  • Informal visits can be arranged on request.
  • We will offer you the choice of two pension schemes; the NHS Pension scheme and the scottish widows group pension scheme.
  • MCH encourages all staff to be double COVID vaccinated to protect themselves, their colleagues, their family and their patients.
